Why do I need to fill out forms? |

A prosthetics and orthotics practice operates very much like a doctor's office and must comply with all the insurance, confidentiality, and regulatory requirements that a doctor’s office does. You may have noticed that the types of forms you are asked to complete here are very similar to what you would complete in a doctor’s office. We promise we will not ask you to fill out a form unless it is required!
